You may also be asked for additional information required to complete and service your clinic registration requests, such as your gender, weight and social security number.
When you complete an online form

How MVN Uses This Information
MVN uses the information you provide to fulfill online requests and shares the information with third parties to complete the request only. Information is not shared with outside, unrelated companies or organizations.
Email Policy
We read every email message that we receive. We also try to
respond within two business days of receipt. If you email MVN, your
message and email address will be forwarded to the appropriate
department within the agency. This department may choose to save
your information in case you decide to contact us again.
